    Golf Check In

    ​Temple Sinai Guest/Visitor Requirements

    ​Because Temple Sinai cannot guarantee zero cases, nor prevent all community infections, we must expect that every member of the community, including guests, will be committed to creating a culture of safety and accountability to maintain a healthy environment.  Everyone who comes to attend this event must understand this reality.  Every guest   must also acknowledge that they have a responsibility to themselves and others to do their part to be safe, honor restrictions, follow appropriate public health guidelines, including those from the Centers for Disease Control, Rhode Island Department of Public Health, and comply with all written Valley Country Club guidance posted or otherwise communicated. 
    • Face Covering: All Individuals are required to wear face coverings until they are on the golf course or otherwise instructed.  All guests are required to provide their own face coverings.

    • Social Distancing: Maintain six feet of distance between you and someone else.

    • Group sizes: All in-person events will comply with current orders from the State of Rhode Island.

    • Hygiene protocols, cleaning and disinfecting: According to the Centers for Disease Control and Prevention, "regular hand washing is one of the best ways to remove germs, avoid getting sick, and prevent the spread of germs to others." 

    • Screening, testing and contact tracing: All individuals must complete the form below prior to each event.

    1. In the past 24 hours, have you developed any of the following symptoms
    • ​A new or worsening cough.
    • Shortness of breath or difficulty breathing.
    • Fatigue.
    • Muscle or body aches.
    • Headache.
    • New loss of taste or smell.
    • Sore throat.
    • Congestion or runny nose.
    • Nausea or vomiting.
    • Diarrhea.
    • Fever > 100.4F
    2. ​Have you been in contact with someone who is suspected (i.e. undergoing or considering testing) to have been infected with Covid 19 or have you been in contact with someone who has tested positive for Covid 19 in the last 14 days?
    3. ​Have you tested positive for Covid 19 in the past 14 days?
    4. Have you traveled out of the country in the past 14 days?
    5. ​Have you traveled out of the state (excluding NE states, NY, NJ) in the past 14 days?
    6. ​Do you prefer individual carts or drive together?

    ​I have read, and understand the Guest and Visitor Requirements.  Furthermore, I (along with any accompanying individuals) completed the self-checklist before this event, and answered no to each question.
